摘要:心肌梗死(Myocardial Infarction, MI)后患者普遍存在睡眠障碍,发生率高达40%-60%,严重影响心脏康复效果与远期预后。运动康复作为心脏康复的基石性干预手段,已被证实能显著改善患者心肺功能、降低再发事件风险。然而,运动康复对MI后患者睡眠质量的影响及其机制尚缺乏系统总结。本文系统回顾了运动康复改善MI后睡眠质量的临床证据,从自主神经功能调节、炎症抑制、心理应激缓解及昼夜节律重塑四个维度探讨了潜在机制,并分析了不同运动类型、强度及实施模式对睡眠改善效果的差异。现有证据表明,以有氧运动为基础的标准化康复方案可有效改善MI后患者的睡眠质量。此外,本文初步探讨了外周血单核细胞中剪切因子I 25kDa亚基(CFIm25)作为新型生物标志物在心肌梗死后心力衰竭预测及康复评估中的潜在价值。然而,高质量的随机对照试验仍较为有限,个体化运动处方的优化及神经生物学机制的深入研究将是未来方向。
